- Download and fill out FCA More Than Baseball Camp Waiver Form
- Email to jbernal@fca.org (prior to December 9th at 5pm) or bring to registration (on December 10th)
- The waiver form MUST be completed in entirety (with guardian signature) in order for your student to participate.
- No waiver form = No participation
